Maritime Accident Case Studies
Course Outline
This awareness program is designed to heighten awareness to Human Factors and Human Error in Maritime Operations. The program contains 20 Maritime accident case studies (15 Large Vessels and 5 Super Yacht Accidents), where human error was the primary cause.
It is important to learn from these errors or events, and make every effort to ensure the same disasters are not repeated. Each case study is analysed using Professor James Reason’s Swiss cheese Model.
