Human error contributes to a significant portion of nuclear incidents, including major accidents like Chernobyl, 3 Mile Island, and Fukushima, and is a leading cause of operational incidents in the industry.
Errors can be active (unsafe actions by operators) or latent (underlying conditions like inadequate procedures or weak safety culture that enable errors). Prevention efforts focus on improving human-machine interfaces, strengthening safety culture, providing better training and procedures, and ensuring leadership commitment to safety to mitigate risks.
